Celem niniejszej pracy jest przedstawienie modelu opieki nad pacjentem z wrzodziejącym zapaleniem jelita grubego (colitis ulcerosa). Jednostka chorobowa należy do grupy nieswoistych zapaleń jelit. Za występowanie Colitis ulcerosa odpowiedzialne są czynniki środowiskowe, genetyczne oraz immunologiczne. Na świecie choruje ok 1,5 mln osób. Przebieg oraz leczenie zależy od stopnia zaawansowania choroby. Innowacyjne leki biologiczne (infliksymab oraz adalimubab) pozwoliły na rozwój nowszych i skuteczniejszych terapii, takich jak step-up, top- down. Objawy choroby mogą być nieswoiste, co może utrudniać diagnostykę. W rozpoznaniu choroby przydatne są badania laboratoryjne, endoskopowe, ultrasonograficzne oraz badanie promieniami X. Metody leczenia są różne i zależą od zmian patologicznych oraz stanu pacjenta. Niewątpliwie ostatecznością są zabiegi operacyjne, a szczególnie te z wyłonieniem sztucznego odbytu (stomii). Oprócz operacji i leczenia farmakologicznego dużą rolę w procesie zdrowienia przypisuje się właściwej diecie. Zarówno podczas leczenia zachowawczego jak i pooperacyjnego.

The paper presents a model of care with ulcerative colitis. Disease entity belongs to a group of inflammatory bowel disease. For the occurrence of Ulcerative colitis are the responsibility of environmental factors, genetic and immunological. In the world suffer from some 1.5 million people. During the acute illness it is essential to use the pharmacotherapy. Innovative biological medicines allowed to improve and develope more efficient therapies like step-up therapy and top-down therapy. Often the symptoms may be non tipical, which doesn’t help the diagnosis. From the proper diagnosis it is useful to do the laboratory tests, endoscopy, ultrasound and X-ray examination. Treatment methods are different and they depend on the pathology of the patient certainly the last step is a surgery, especially those with the emergence of an artificial anus (stoma) In addition to surgery and medicines therapy major role in the recovery process is attributed to diet. A very important role is given to a correct diet. This is because of it’s benefits during the treatment and post- operative as well.
