Hepatology Group of Polish Society of Gastroenterology recommendations for cholestatic liver diseases : adaptation of European guideline

Celem publikacji jest przybliżenie polskim lekarzom europejskich wytycznych dotyczących diagnostyki i leczenia chorób przebiegających z cholestazą. Wytyczne europejskie przedyskutowano na forum Sekcji Hepatologicznej Polskiego Towarzystwa Gastroenterologii pod kątem możliwości ich zastosowania w polskich realiach, a następnie poddano głosowaniu przez członków Sekcji. W opracowaniu przedstawiono ogólne zasady diagnostyki cholestazy oraz omówiono wytyczne rozpoznawania i leczenia najczęstszych chorób przebiegających z cholestazą — pierwotnej żółciowej marskości wątroby, pierwotnego stwardniającego zapalenia dróg żółciowych, zespołów nakładania, polekowych cholestatycznych uszkodzeń wątroby i zespołów cholestazy występujących u ciężarnych i dzieci. Omówiono również IgG4-zależne stwardniające zapalenie dróg żółciowych — niedawno poznaną i często sprawiającą trudności diagnostyczne jednostkę należącą do spektrum układowej choroby IgG4-zależnej. Po każdym rozdziale przedstawiono komentarz panelistów odnoszący się do rozbieżności w głosowaniu.

The main purpose of this paper is to acquaint Polish practitioners with current European guidelines on diagnosis and management of cholestatic liver diseases. The European guidelines proposed by EASL experts has been discussed in several aspects by members of Hepatology Section of the Polish Society of Gastroenterology. Panelists, all active in the clinical hepatology, debated on usefulness and possibilities of implementation of these guidelines in Polish realities. As a final point, panelists voted on the level of acceptance of European guidelines for their realization in our country. The paper presents the general approach to the cholestasis and current recommendations on the diagnosis and treatment of most frequent cholestatic liver diseases such like primary biliary cirrhosis, primary sclerosing cholangitis, overlap syndromes, drug-induced cholestatic liver injuries and cholestatic diseases occurring in pregnancy and childhood. A newly recognized IgG4-related sclerosing cholangitis, the condition included into wide spectrum of systemic IgG4-related disease, was also presented. Each section was followed by a brief comment explaining discrepant voting results.
