Nursing problems in the care of patients with rheumatoid arthritis.

Wstęp: Reumatoidalne zapalenie stawów (RZS) jest przewlekłym postępującym procesem zapalnym błony maziowej, doprowadzającym do niszczenia tkanek stawowych i okołostawowych, w wyniku czego dochodzi do zniekształceń i upośledzenia czynności tych tkanek, a w konsekwencji do trwałego kalectwa. Celem leczenia i pielęgnacji chorych z RZS jest zmniejszenie objawu sztywności porannej stawów, zwiększenie siły mięśniowej rąk, zahamowanie uszkodzeń stawów, zapobieganie utracie funkcji stawów i niepełnosprawności, jak najdłuższe utrzymanie aktywności życiowej i zdolności do pracy oraz spowodowanie remisji choroby.Cel pracy: Celem pracy było przedstawienie problemów zdrowotnych aktualnych i potencjalnych u pacjentki chorej na RZS przebywającej w domu oraz planu interwencji pielęgniarskich. Materiał i metody: Praca ma charakter kazuistyczny, napisana została w oparciu o metodę studium przypadku i metodę badań opartych na zgromadzonych dokumentach. Wyniki: Problemy zdrowotne, które wystąpiły u chorej z RZS, wynikały z oceny stanu fizycznego, psychicznego i społecznego. Dotyczyły zniekształceń rąk i stóp, obniżenia siły mięśniowej rąk, silnych bólów stawowych, lęku spowodowanego postępem choroby, ograniczenia ruchomości stawowej oraz owrzodzeń żylakowych kończyn dolnych. W planie pielęgnacyjnym, jako ważny element interwencji uznano edukację zdrowotną w zakresie samoopieki i przygotowania otoczenia zapewniającego bezpieczeństwo chorej.

Introduction: Rheumatoid arthritis is a chronic, progressing inflammatory process of synovial membrane leading to the destruction of articular and periarticular tissues which results in deformation and impairment of these tissues and finally in permanent invalidism. The aim of treatment and care of the patients suffering from this disease if to decrease the symptoms of morning stiffness of joints, to increase the muscle power in arms, to restrain the joint damage, to prevent the loss of joint functions and disability, to keep the patient in good condition- suitable for work- as long as possible and to achieve the remission of the illness.The aim of work: The aim of work was to present the health problems, both real and potential, of a patient suffering from this disease staying at home, together with a plan of nursing intervention.Material and methods: The work has a casuistic character; has been written basing on the method of a case study and the method of research based on collected documents.Results: The health problems associated with the patient mentioned above were generated by the estimate of her physical, psychological and social condition. They concerned deformation of hands and feet, decrease of arm muscle power, strong joint pain, fear of advancing illness, limitation of joint mobility and varicose ulcer of lower limbs. One has considered the health education as an important element of nursing plan. It should include self-care and preparation of surroundings which ensures the patient`s safety.
