Nursing home care of patients with nephrostomy.

Nefrostomia to przetoka moczowo-skórna, która jest wykonywana w obrębie nerki. Celem jej jest odprowadzanie wyprodukowanego w nerkach moczu, z chwilą kiedy jego naturalny odpływ nie jest możliwy. Na wykonanie zabiegu ogromny wpływ ma rodzaj jednostki chorobowej, jej zaawansowanie, stopień uszkodzenia, bądź upośledzenia dróg moczowych, oraz ogólny stan zdrowia pacjenta.Celem niniejszej pracy było rozpoznanie problemów zdrowotnych choregoz nefrostomią, a także opracowanie modelu opieki pielęgniarskiej z zastosowaniem teorii deficytów samoopieki według modelu D. Orem. Model ten ma ułatwić i usprawnić pracę pielęgniarkom sprawującym opiekę nad wyżej wymienionymi pacjentami. Praca składa się z czterech części. W części pierwszej dokonano przegląd literatury przedmiotu na temat nefrostomii, podano definicję, jednostki chorobowe, wskazaniai przeciwwskazania oraz technikę i powikłania związane z tym zabiegiem. Część druga pracy przedstawia założenia i zadania pielęgniarki w opiece domowej.Część trzecia pracy odnosi się do metodologii badań własnych, w której przedstawiono cel pracy, materiał i metodę.W kolejnej czwartej części omówiono model opieki według D. Orem, który zastosowano w opiece na pacjentem z nefrostomią, scharakteryzowano 13 problemów zdrowotnych pacjentów z nefrostomią oraz metody ich rozwiązywania. Pracę zakończono podsumowaniem i wnioskami, wykazem piśmiennictwa, z którego zaczerpnięto informacje na temat nefrostomii.

Nephrostomy is a cutaneous fistula tract, which is executed within the kidneys. Its purpose is to remove the urine produced by the kidneys, when the natural drainage is not possible. The procedure of nephrostomy development is influenced by the type of disease entity, its severity, degree of injury, or impairment of urinary tract, and the general health of the patient.The aim of this study was to assess the patient's health problems with nephrostomy, as well as to develop a model of nursing care using the self-care theory deficits by model D. Orem. This model aims to facilitate and improve the work nurses who have the care of the above-mentioned patients.The work consists of four parts. In the first part a literature review was made according to nephrostomy, the definition, disease entities, indications and contra-indications and technique and complications associated with this procedure.The second part of the work presents the assumptions and tasks of nurses in home care.The third part of work refers to the methodology of its own, which presents the aim, material and methods.In another fourth part discusses the model of care by D. Orem, which applied in the care for the patient with nephrostomy, characterized 13 health problems of these patients and methods of solving them.The work was completed with summary and conclusions, list of references, which derived information about the nephrostomy.
