Patient care during treatment of papillary thyroid carcinoma - case study

Wstęp: Wśród chorób nowotworowych tarczycy najwyższy odsetek zachorowań posiada rak brodawkowaty tarczycy. Jakkolwiek, występuje on częściej u kobiet to z każdym rokiem wskaźnik zachorowań wzrasta dla obu płci na całym świecie. Osoby cierpiące na tę chorobę w początkowym stadium nie odczuwają uciążliwych dolegliwości, dlatego bagatelizują je i rezygnują z wizyty u lekarza, co wpływa na diagnostykę oraz leczenie. Każdy pacjent wymaga wysokospecjalistycznej opieki pielęgniarskiej, która jest stałym elementem leczenia na każdym etapie choroby.Cel pracy: Celem pracy było przedstawienie problemów zdrowotnych oraz zakresu opieki nad pacjentem w trakcie leczenia raka brodawkowatego tarczycy. Materiały i metody: W pracy zastosowano metodę studium indywidualnego przypadku oraz następujące techniki badawcze: analiza dokumentacji medycznej i wywiad pielęgniarski. Opisano historię choroby 33-letniej pacjentki z rakiem brodawkowatym tarczycy. Wyniki i wnioski: U pacjentki niepokojące objawy wystąpiły w trzeciej dekadzie życia. Pierwszym objawem był ból brzucha oraz mdłości, przewlekłe uczucie zmęczenia, uciążliwe bóle głowy, zatrzymanie miesiączkowania. Pacjenta przebyła leczenie chirurgiczne oraz radiojodoterapię. Obecnie jest objęta leczeniem ambulatoryjnym. W pracy sformułowano 16 diagnoz pielęgniarskich, które dotyczyły poszczególnych etapów leczenia. Do każdej diagnozy określono cel opieki, interwencje pielęgniarskie oraz uzyskany wynik. Praca ma ułatwić pielęgniarkom sprawowanie opieki nad chorymi z tą jednostką chorobową.

Introduction:Among different types of thyroid neoplasmatic disease, the one with highest incidence rate is papillary thyroid cancer. However, it occurs more frequently in women, each year the rate of incidence grows for both sexes all over the world. People suffering from this disease in the early stages do not feel oppressive symptoms, so they don't decide to consult a doctor, what influences diagnostics and the treatment. Each patient requires highly specialized nursing care, which is an integral part of treatment at each stage of the disease. Aim:The aim of this study was to present health problems and the scope of care of a patient treated because of papillary thyroid cancer. Materials and methods:This paper describes a medical history of a 33-year-old female patient diagnosed with papillary thyroid cancer. The method of individual case, as well as the following research techniques: medical records analysis and nursing anamnesis was used.Resutls and conclusion:Alarming symptoms occurred in the third decade of patient's life. The first was stomach ache, as well as nausea, chronic tiredness, oppressive headache and amenorrhoea.Patient was treated with surgery and radioiodine therapy. Currently, she is under ambulatory care. Sixteen nursing diagnoses related to different stages of the treatment were formulated in this paper. For each of them there were defined: the goal of care, nurse interventions and end result. This work aims to make it easier for nurses to take care of patients diagnosed with this disease entity.
