Opis przypadku pacjenta ze stwardnieniem rozsianym

Aim. The aim of the thesis was to show the course of multiple sclerosis on the basis ofcase-study and to identify the most important problems which are faced by the patient,to plan nursing interventions and evaluate its results.Materials and methods: The case study method was used - analysis of medicalrecords, interview and observation of the patient.Results: The natural course of multiple sclerosis on the basis of case study was shown.Twelve nursing diagnoses were identified, nursing interventions were planned andnursing outcomes were evaluated.Conclusions: The first symptom occurred in a patient 23 years ago. Now the degree ofdisability of the patient is very advanced. The patient have multiple health problems,both in the physical and mental.

Cel pracy. Celem pracy było ukazanie przebiegu stwardnienia rozsianegow zaawansowanym stadium na podstawie opisu przypadku pacjenta orazzidentyfikowanie jego problemów, zaplanowanie interwencji i ocena wyników opieki.Materiał i metody. W pracy zastosowano metodę indywidualnego przypadku.Wykorzystano technikę analizy dokumentacji medycznej, wywiadu i obserwacjipacjenta.Wyniki. W pracy ukazano naturalny przebieg stwardnienia rozsianego na podstawiestudium przypadku pacjenta. Zidentyfikowano 12 diagnoz pielęgniarskich,zaplanowano interwencje pielęgniarskie i oceniono wyniki opiekiWnioski: Pierwszy rzut choroby wystąpił u pacjenta 23 lata temu. Obecnie stopieńniepełnosprawności chorego jest bardzo zaawansowany. U pacjenta występuje wieleproblemów zdrowotnych, zarówno w sferze fizycznej jak i psychicznej.
