Opieka nad pacjentką z serododatnim reumatoidalnym zapaleniem stawów w stadium III

Wstęp: Reumatoidalne zapalenie stawów jest przewlekłą jednostką chorobową o charakterze postępującym, a jej przyczyna nie jest do końca poznana. Objawy są zróżnicowane i obejmują zmiany zarówno stawowe jak i pozastawowe. Szacuje się, że występuje u ok. 0,3 – 1,5% osób w krajach europejskich, przy czym częściej u kobiet. W przebiegu choroby niezbędne jest stosowanie zarówno farmakoterapii, jak i rehabilitacji. Przyczynia się to do zmniejszenia nasilenia objawów chorobowych. Jednak u większości chorych następuje postęp choroby utrudniający funkcjonowanie i powodujący niepełnosprawność prowadząc z czasem do przedwczesnej śmierci. Cel: Celem pracy była ocena funkcjonowania chorej w przebiegu reumatoidalnego zapalenia stawów w stadium III o charakterze seropozytywnym, a także zidentyfikowanie jej aktualnych problemów zdrowotnych i na tej podstawie opracowanie planu opieki.Materiał i metody: Prace oparto o studium przypadku pacjentki z serododatnim reumatoidalnym zapaleniem stawów w stadium III, używając do tego kategoryzowanych skal oraz kwestionariusza wywiadu.Wyniki i wnioski: U pacjentki utrzymują się dolegliwości bólowe oraz objawy przedmiotowe choroby. Zastosowana różnorodna farmakoterapia nie przyniosła oczekiwanych efektów, obecnie czeka ona na włączenie leczenia biologicznego. Opracowany plan opieki został wdrożony w życie, dzięki interwencjom pielęgniarskim oraz edukacji jakość życia chorej uległa poprawie, zmniejszyło się również nasilenie objawów chorobowych.

Introduction: Rheumatoid arthritis is a chronic, progressive disease, and its cause is not fully understood. Symptoms vary and include both joint and non-articular changes. It is estimated that it affects approximately 0.3 - 1.5% of people in European countries, with more frequent cases of women. In the course of the disease, it is necessary to use both pharmacotherapy and rehabilitation. This contributes to reducing the severity of disease symptoms. However, in the majority of patients the disease progresses impeding functioning and causes disability, leading overtime to premature death .The Aim: The aim of the study was to assess the patient's functioning in the course of rheumatoid arthritis in stage III with seropositive character, as well as to identify its current health problems and, on this basis, to develop a care plan. Data and methods: The work was based on a case study of a patient with seropositive rheumatoid arthritis in stage III using categorized scales and an interview questionnaire.Results and conclusions: The patient suffers from pain and the symptoms of the disease are still visible. The various pharmacotherapy used did not bring the expected effects, now the patient is waiting for the inclusion of biological treatment. The care plan has been implemented, thanks to nursing interventions and education, the quality of patient’s life has improved, and the severity of disease symptoms has been reduced.
